Any non-UC staff or student must sign a waiver especially if going out in a small boat, using the pier, or similar.

Just need to fill out the form below and be sure to list the activity in the appropriate box. Have the party sign & date and return to the IOD Business Office (Jennifer). The department is responsible for keeping the releases;
